On 16 July 2025, six cars were aboard the cable ferry Paule II. After a two-minute crossing, the drivers were instructed to disembark. As the third and fourth cars drove over the shore ramp, a gap gradually opened between it and the vessel.

On 03 July 2025, the general cargo vessel Kathy McKeil was approaching the Superior Elevator dock in Thunder Bay, Ontario. During mooring operations, a crew member was being transferred to the dock using a landing boom—an apparatus designed to transfer personnel from the vessel to shore. During the transfer, the crew member handling the landing rope lost his grip, causing the other crew member to fall into the water.
On 05 June 2025, at approximately 1520 Pacific Daylight Time, the master and a deckhand on the landing craft Duu Guusd were using a skiff to go from shore to the moored Duu Guusd when the skiff capsized off Kiusta, Haida Gwaii, British Columbia. Both the master and the deckhand fell into the water, which was approximately 10 °C. Both were wearing personal flotation devices.
On July 23, 2025, the Transportation Safety Board of Canada (TSB) launched a safety issue investigation (SII) (M24A0348) into shipboard fires on commercial vessels and the effectiveness of emergency response in Canada.
Unlike individual occurrence investigations, an SII is broad in scope, involving an in-depth review of multiple occurrences and consultations with key industry stakeholders.
