Aviation Investigation Report A16P0045

The occurrence

On 16 March 2016, an Airbus Helicopter AS350 BA, operated by TRK Helicopters was operating on a heli-skiing flight from the Bear Creek Lodge, British Columbia, about 82 nautical miles northwest of Smithers, British Columbia. The flight was returning to base camp when the helicopter collided with terrain on a steep snow covered slope. The helicopter was substantially damaged, but the emergency locator transmitter was not triggered to send out an emergency signal.

The occurrence

On 30 January 2016, an Air Canada Embraer 190 (ACA726) was taxiing for departure on Runway 24R at Toronto’s Lester B Pearson International Airport (LBPIA). ACA726 taxied onto the runway without a clearance while an Air Canada Airbus A320 (ACA1259) was on short final for landing on Runway 24R. ACA1259 initiated a missed approach as a result of the incursion.

The occurrence

On 4 September 2016, a private Bell 206B helicopter departed Charlo, New Brunswick, for Rivière-du-Loup, Quebec, in visual flight rules weather conditions. The helicopter collided with power lines near Flatlands, New Brunswick, causing substantial damage, and fell into the Restigouche River. The pilot and the front passenger were fatally injured. The rear-seat passenger survived the accident.

The occurrence

On 29 March 2016, a private Mitsubishi MU-2B-60 aircraft (United States registration N246W), departed St. Hubert, Quebec, at 0931 local time (Eastern Daylight Time) on a flight to Îles-de-la-Madeleine, Quebec (CYGR). Onboard was the pilot-in-command, a pilot-passenger occupying the right-hand cockpit seat, and 5 passengers.

The autopilot was being used to control the aircraft throughout the flight.
