Air transportation safety investigation A16W0094

The occurrence

In July 2016, a North American Inc. T28-B aircraft, privately registered as C-GKKD, was taking part in an air show at Canadian Forces Base Cold Lake (CYOD), Alberta. On July 17, the aircraft impacted terrain during an aerobatic routine, and the pilot was fatally injured. There was no post-impact fire and no injuries to airshow personnel or spectators. There was minor damage to airfield infrastructure.

The occurrence

On 24 May 2016, a De Havilland DHC-2 Beaver, C-FJOM, operated by Inland Air Charter was landing at the water aerodrome in Kitkatla, British Columbia, when the aircraft landed hard, bounced, and landed hard again. The left wing made contact with the water. The aircraft settled in the water and partially nosed over. There were 7 person on board; all egressed into the water as the aircraft sank.

The occurrence

On 4 May 2016, a Highland Helicopters Bell 206 B departed Williams Lake (CYWL), British Columbia, and was carrying out scanning operations over a logged area about 112 nm NW of Prince George, British Columbia, when it spun to the right and descended into the ground. The helicopter was destroyed and the pilot was injured, but the two passengers appeared to have no injuries. All three were taken to hospital in Fort St. James, British Columbia. There was no post-impact fire.
