Air transportation safety investigation A17P0007

The occurrence

On , a Victoria Flying Club Cessna 172 was conducting a training flight departing from Victoria Airport, British Columbia, with one student pilot and one instructor on board. During the flight, the aircraft struck power lines and crashed off the north end of Runway 31 at Duncan Airport, British Columbia. The instructor sustained serious injuries and the student pilot sustained minor injuries.

The occurrence

On 15 December 2017, a privately operated Piper PA-23-250 aircraft was conducting a flight from Gillam, Manitoba, to a private aerodrome 5.2 nautical miles (nm) east of Baldur, also in Manitoba. The aircraft collided with terrain 0.36 nm southwest of the intended destination. The pilot, who was the sole occupant, sustained fatal injuries. There was no post-impact fire, but the aircraft sustained substantial damage. The TSB is investigating.

The occurrence

On 5 September 2016, a Ridge Rotors Inc. Bell 206B helicopter was conducting a wildlife survey flight that had originated in Whitecourt, Alberta. During the flight, the helicopter experienced a power loss, contacted trees, and struck the ground. The pilot sustained serious injuries; one passenger had minor injuries; and one passenger was fatally injured. The helicopter was substantially damaged.
