Air transportation safety investigation A22O0140

The occurrence

On at 1248, a privately registered Quad City Challenger II advanced ultralight aircraft departed a private runway near Yarker, Ontario, for a flight to Westport/Rideau Lakes aerodrome. During the flight, the aircraft entered an uncontrolled descent after the engine lost power, and collided with terrain in a wooded area. The pilot, who was the sole person on board, was fatally injured.

The occurrence

On , at 0013 Eastern Daylight Time, during the hours of darkness, the maintenance vehicle designated as Maintenance 31, operated by the Greater Toronto Airports Authority at Toronto/Lester B. Pearson International Airport, Ontario, crossed the runway holding position marking for the east side of Runway 15R on Taxiway J, even though the driver had received and read back a taxi clearance that included an instruction to hold short of the runway.
