Air transportation safety investigation A24P0016

The occurrence

On 07 February 2024, a British Aerospace Avro 146 Series RJ100 aircraft, operated by Summit Air Ltd., was conducting a flight from Nanaimo Airport, British Columbia (BC), to Prince Rupert Airport, BC, with 4 crew members and 34 passengers on board.

After touchdown, the aircraft overran the runway, coming to a stop approximately 30 m beyond the threshold of Runway 31, facing northeast.

The occurrence

On , an Aerospatiale AS 350 BA helicopter, operated by Aberdeen Helicopters Ltd., took off from Vernon Airport, British Columbia, on a visual flight rules training flight, with 2 crew members and 1 passenger on board.

After a short time in flight, the helicopter’s hydraulics were shut off to accommodate the training exercise. A few minutes later, the helicopter began to pitch nose-down.

The occurrence

On 28 June 2024, a Bell Textron Inc. 212 helicopter operated by Great Slave Helicopters 2018 Ltd. was in the area of Fort Good Hope, Northwest Territories, supporting wildfire suppression efforts. Shortly after takeoff from Fort Good Hope Airport (CYGH), the helicopter experienced a main rotor blade separation from the head. Immediately thereafter, the remaining rotor blade and rotor head separated from the mast. The helicopter then collided with the terrain, resulting in a post-impact fire.
