The Australian Transport Safety Bureau (ATSB) investigated an accident on 18 November 2009, in which a Westwind 1124A ditched near Norfolk Island, Australia. Questions were raised in the media about the final investigation report, and an Australian Senate Committee inquiry yielded a number of unfavorable findings and recommendations for change.
