  • Hard landing and runway excursion of Maritime Air Charter Limited, Beechcraft King Air A100, C-FDOR at Margaree, Nova Scotia on 16 August 2015

    On 16 August 2015, a Maritime Air Charter Limited Beechcraft King Air A100 (registration C-FDOR, serial number B-103) was on a charter flight from Halifax Stanfield International Airport, Nova Scotia, to Margaree Aerodrome, Nova Scotia, with 2 pilots and 2 passengers on board. At approximately 1616 Atlantic Daylight Time, while conducting a visual approach to Runway 01, the aircraft touched down hard about 263 feet beyond the threshold. Almost immediately, the right main landing gear collapsed, then the right propeller and wing contacted the runway. The aircraft slid along the runway for about 1350 feet, then veered right and departed off the side of the runway. It came to rest about 1850 feet beyond the threshold and 22 feet from the runway edge.

  • Striking of ice and subsequent flooding of the fishing vessel Saputi in the Davis Strait, Nunavut on 21 February 2016

    On 21 February 2016, the fishing vessel Saputi, with 30 people on board, was fishing turbot in the Davis Strait, 167 nautical miles east-northeast of Resolution Island, Nunavut, and 220 nautical miles west-southwest of Nuuk, Greenland. At 1935 Atlantic Standard Time, the vessel struck a piece of ice and was holed in the shell plating on the starboard side at the forward end of the cargo hold. After pumping operations failed to keep up with the ingress of water, the cargo hold was sealed, and it subsequently flooded. The vessel developed a severe list to port but proceeded under its own power to Nuuk, Greenland, arriving on 24 February. No injuries were reported.

  • Loss of directional control and collision with terrain of 6442927 CANADA INC. (operating as Héli-Nord), Bell Helicopter 206B, C-GYBK, 20 nm N of Sept-Îles, Quebec on 02 September 2015

    On 02 September 2015, the Bell 206B helicopter (registration C-GYBK, serial number 1884) operated by Héli-Nord was flying from the airport of Sept-Îles, Quebec, with 1 pilot and 4 passengers on board. The purpose of the flight was to inspect a salmon pass approximately 20 nautical miles north of Sept-Îles. During the final approach, a few feet from the ground, the helicopter began an uncommanded rotation to the right and, after turning a few times, crashed heavily into a rock on its front right side. The accident occurred at about 0940, Eastern Daylight Time. The male passenger occupying the front left seat and the female passenger occupying the rear central seat sustained fatal injuries. The pilot and the other 2 passengers, who occupied the left and right rear seats, sustained serious injuries. The 406-MHz emergency locator transmitter activated on impact. A fire started in the engine tailpipe but was immediately extinguished by persons on site.

  • Main-track train derailment of Canadian National Railway Company freight train M31331-07 at mile 13.45, Redditt Subdivision, Webster, Ontario on 09 January 2016

    On 09 January 2016, at 0802 Central Standard Time, Canadian National Railway Company freight train M31331 07 was proceeding westward on the Redditt Subdivision at about 46 mph when a train-initiated emergency brake application occurred at Mile 21.74, near Webster, Ontario. Subsequent inspection determined that a total of 26 cars (29 platforms) had derailed. The derailed cars included 6 Class 111 dangerous goods residue tank cars that had last contained diesel fuel (UN 1202). There were no injuries and no product was released.

  • Runway excursion of Air Canada Airbus A330-343, C-GFAF at Montréal/Pierre Elliott Trudeau International Airport, Montréal, Quebec on 07 October 2014

    On 07 October 2014, the Air Canada Airbus 330-343 (registration C-GFAF, serial number 0277), operating as flight number ACA875, left the Frankfurt-Rhein/Main International Airport in Germany, bound for Montréal/Pierre Elliott Trudeau International Airport, Quebec. During the daylight approach to Runway 24R in the presence of a thunderstorm just north of the airport, the crew was advised that the lighting on Runway 24R was out of service. This was the only runway in operation. During final approach, meteorological conditions changed from visual meteorological conditions to instrument meteorological conditions. On short final, during the approach to Runway 24R, using the instrument landing system, the aircraft entered a heavy rain shower and encountered a right crosswind exceeding 20 knots. The aircraft deviated from its path before touching down to the left of the runway centreline at 1234 Eastern Daylight Time. Following the landing, the left rear tire of the left main landing gear left the hard surface of the runway and travelled approximately 600 feet before regaining the runway. The aircraft returned to the centreline before taxiing to the terminal gate, where the passengers exited. Three runway edge lights were damaged. The aircraft was not damaged and there were no injuries.

  • Uncontrolled movement of railway equipment of Cando Rail Services, Co-op Refinery Complex at mile 91.10, Canadian National Railway Company, Quappelle Subdivision, Regina, Saskatchewan on 01 March 2016

    On 01 March 2016, at approximately 2335 Central Standard Time, the Cando Rail Services 2200 Co-op Refinery Complex assignment was switching tank cars loaded with asphalt at the Co-op Refinery Complex in Regina, Saskatchewan. At 2347, tank car GATX 67963 rolled away uncontrolled. The car travelled about 2.7 miles (4.3 kilometres), reaching a speed of 19 mph. The car traversed 7 public crossings at grade, each protected by automatic warning devices, and 1 railway interlocking (diamond) that crossed the Canadian Pacific Railway's Lanigan Subdivision before coming to rest in the city of Regina. There were no injuries, and no dangerous goods were involved.
